Schedule An Appointment

We look forward to having you as one of our patients and taking care of your eye needs.  To schedule an appointment, please call our office at 414.328.8760.


 On the day of your appointment, please bring: 

* A list of your medications, eyedrops, and supplements

* Your glasses, contacts, and sunglasses (your eyes may be dilated)

* Insurance cards, including prescription cards

* The name, address, and phone number of your pharmacy

* Eye medical records (for new patients)

* Referrals should be made prior to the time of the exam.

Since, in most cases, you will be having a complete medical eye exam, please allow between 1 and 1 ½ hours for your appointment. Your pupils may be dilated, so bring your sunglasses! If your eyes are sensitive to light, you may want to bring a driver. Your vision will be blurred for any close work for 3 to 5 hours after being dilated.

Co-payments and payments for services that may not be covered by insurance are due at the time of the appointment.  
We accept cash, checks, and credit cards
A word about “refraction.” Refraction means checking what your eyeglass prescription is. This is necessary to determine what your best vision is and if you need new glasses. Medicare and many other insurances, including secondary carriers, do not consider this a covered service, but still require it to be done before any surgical procedure is scheduled.  Also, there are a number of medical conditions that require the refraction to be done so that the doctor has all the information she needs to best evaluate and treat you.  This is a separate charge from the eye exam, and we will ask for payment of $40 at the time of service.  Again, Medicare and some other insurance plans have never covered this service, but it is impossible to give you a full evaluation without doing the refraction.
